404 after form submission

You cannot have a custom field named “name.” It will cause a 404 error – the “Oops! We can’t find what you’re looking for” page.

Use the default “client_name” or something like “first_name”, just not “name” by itself. However, you can change the field label as you see fit: “Name”, “Your Name”, “First Name”, etc.

I will work on catching this in the plugin.
