4 Forms and Form 2 is Registering All Entries to Form 1

Okay, I am at a loss and have tried renaming slugs and everything. I set up four forms. They are all similar, though a bit different, so I cloned the first one to build forms 2, 3 and 4, then made required changes.

I setup custom emails to be sent when forms are submitted to our office manager. Forms 1, 3 and 4 work, but Form 2 registers the entries to Form 1, instead of the Form 2 entry list, and sends the email for Form 1. Although it handles the redirect upon submission to the correct place with no problem.

I went back and changed all the slugs for the entry fields, thinking there was a conflict and it is still doing it. Any thoughts or suggestions? ??
