    last upgrade may have broken the form. It works fine on non-networked site. Any ideas how to fix it?

    where to find 3.7.0 version?

    When you say broken what do you mean? I’m more trying to understand as we may have different meanings.

    Yes, I have. I don’t see anything in the error logs. All plugins deactivated,

    Same problem. Rolled back to 3.7.0 and form works.

    Looking at changelog and console errors I’d say it’s due to “Update JS libraries”
