2 or more different forms?

Love this plugin, but can I make another form with the category and other fields different from the first? is there a call function and shortcode so I can add more than one forms on different pages?

Or I could change the name and somehow make 2x WP user frontend in my plugin list and use one for each form?

basically I need 2 or more different forms how can that be achieved?
