2 of 5 fields not received my recipient Email

The user can submit all 5 fields, but the recipient email only receives the first 3 fields that were sumbitted. Recipient doesnt receive firlds 4 and 5 that is submitted by the user.

I receive the form submission, but not 2 of the 5 fields that are submitted.
