2 of 3 forms work. Red Box. How to troubleshoot

The Contact Us form and a survey form work.
The to and from fields are the same for a 3rd form, but I get the red box border error that it failed to send.
I am not receiving the email either.
What steps can be taken to troubleshoot this?
