2 forms on one page duplicates form input field entries!

I have 2 forms on one page
One is a the top asking folks to sign up to d/l a report
the second is in the footer, asking for a newsletter sign up.
When I put content in the footer form, the same content shows up in the Get a report form
it’s the same thing, first/last/email
