1.9.5 to 1.9.7 not working

Trying to upgrade from 1.9.5 to 1.9.7 and doing so completely breaks the page where the map is rendered.
The new version adds a search bar above the map where previously I had none (in addition to a dropdown for radius and a search button). Additionally, all the results are showing NULL for both City and State and using the newly added search field yields a warning dialog that says “not found” no matter what is input in the search field.
