1 Form for Multiple Pages

I am working on an animal care guide website. I want to create a form where users can input their past experiences with different animal species. Is there a way to create one form that can be used across multiple care guides that will store submitted information from each animal individually?
As an example, let’s say I have created a form called “Bird Care”. With that, I have care guides on Macaws, African Gray Parrots, and Lovebirds. Each bird species has its own page with information. Is there a way for me to add the “Bird Care” form to each page, but when users submit the information from the form, the data from the form is stored separately for each bird? I want to create graphs and charts that correlate with form submissions on each bird.
