Hi after update form 1.9.11 to 1.9.12, at cart page i have :

I have oldest version 1.9.11 and this version works well,
where i can find changelog?

Please make sure that you are using PHP version 5.6 or higher.

Also, I can see that you have changed your plugin folder name. Please don’t this/ It can harm your site.
This is wrong:
/wp-content/plugins/flexible-shipping-old/This is correct:
/wp-content/plugins/flexible-shipping/Make sure that the folder name is always the same as in zip package.
