    Do you have the “Script Load Method” in the troubleshoot area as “Direct” or “Classic”? Please change that setting and let me know the result.

    I’ve been using the Direct method. But I’ve tried both and neither work.

    It may be a conflict with the autooptimize plugin. Could you temporary disable it to check if it is the one causing the conflict?

    I deactivated AutoOptimize. That didn’t solve the problem. I’ll try to deactivate a few other plugins and let you know.

    same issue here:
    1.1.92 works fine, after updating to 1.1.93 the form is not shown.
    After installing 1.1.92 again, anything is ok.

    I’ve published a new update 1.1.94. After the update you may have to clear the browser cache to reload the updated script.
