    このようなエラーが出てしまいフォームが表示できません。

    こちらはどのように修正すれば宜しいでしょうか？

    エラーメッセージの添付ありがとうございます。回答しやすいです。

    404エラーが起こったときのテンプレートを読み込もうとして失敗しているようです。

    テンプレートを用意できそうなら、用意すると改善するはずです。
    用意できない場合は、テーマを変えてみると改善する見込みが高いです。

    それとは別に、404エラー時のテンプレートが見つからないときにAFormsがエラーメッセージを出すのは良くないので、これは改善しようと思います。
