Checkmarks not displaying in PDF

I have checkmarks in my form, but if I tick boxes and submit the form, the form comes through in the PDF, but does not display the checkmarks. How do I correct this?

I have created a Form, with three “agreement” tick boxes.

When I look at the individual Subscriber’s notes, there are multiple fields showing.
Not use what I have done to create this effect.

I would expect to make three tick boxes, and see three matching boxes in the back-end!?

Hello!

Hopefully someone can help me. In the last few days, possibly due to an update, none of my check boxes are working throughout the site.

They don’t work in the admin side, woo-commerce, or any other part of the site. Can anyone help, as this is causing me huge problems with simple things like creating variable products, as so much requires a tick box somewhere along the line.

Literally not one tick box works. When I hover over them, I get the little hand rather than an arrow, but when pressing, nothing gets ticked or selected.

Under Marketplace->Settings->Marketplace Settings, I’m not able to tick any boxes such as “Product Approval” or “Visible Sold By”. Does anyone know what the issue might be? Screenshot is below.

Tick boxes for product variations

Hi,
I want to be able to add several additions to my products as variations with Woocommerce. However I don’t want them to be drop down boxes for the user to select, I want the variations to be a list of tick boxes, with each one adding a certain amount on to the price. How do I do this?

Cannot check/uncheck tick boxes in widgets on sidebar

Don’t seem to be able to check/uncheck any tick boxes for any of the widgets I use on the sidebar, which is pretty annoying.

Have tried on a different PC, and the same problem.

Using bueno theme.

Is this a fault with wordpress? Can it be easily rectified?
