1. After selection a desired radio button, the next Tab press goes to the input field of the last radio button, which is unexpected, because the selection of the desired radio button changes to the “Custom” radio button.
Expected Result:? When using the Tab key to navigate among different fields, the selected radio button is expected to remain unchanged.
2. On the Posts page under Screen Options, the radio buttons are announced as “1 radio button checked” rather than “Number of Columns. One radio button checked”.
Expected Result: “Number of Columns” should be available for JAWS to announce.
3. Several pages within WordPress have missing tab stops.? For example, the PlugIn page is missing the “Activate your Akismet account” button.? Another example is the tab stop for the “Add Media” button in the “Add New Post” page.
Expected Result:? All user interface elements should be navigable via the keyboard. In this instance, the “Activate your Akismet account” button should have a tab stop available for a keyboard user.?

]]>Then I restore the wp-includes folder and it’s up again…for a short while. I removed all unused plugins, I update to WordPress 3.5 – I even installed the OSE FIREWALL plugin and it still keeps going down.
Am I being hacked — or is there something faulty in the code here that’s bringing it down? I already contacted the theme builder and they said since the error comes from a WordPress file, I should ask here.
Can anyone tell me what’s happening? Or if there is a particular plugin that gets targeted? All of our other WordPress sites work.

I’m not sure without looking thru my past posts on these forums whether you’ve helped me directly (but I’m pretty sure you have, even recently I think), but you have certainly helped me with your answers to other people.
Again and again, when searching for a solution to a WP problem I can’t quite get my head around, I have found posts where you have posted a solution and/or an explanation of some thorny or poorly documented aspect of WordPress.
You post so often that you must log in a lot of hours doing this, and yet you are always straightforward in your answers without any snarky attitude.
You frequently post specific, relevant code as an example (instead of just directing people to “look it up”), and I have often saved those snippets of code for future reference.
So that’s all. You can delete this post if you want. I just wanted to tell you that I have logged many, many hours in these forums for the last couple of years, and you are real asset to the community.
PS Ah, while I’m at it, I should also say that the user “Esmi” is also a tremendously helpful resource, and deserving a post of (his/her?) own, but, crap, I gotta get back to work.

esmi kindly pointed me to the correct WordPress FAQ troubleshooting section (here). However, I’ve tried the common solutions to the headers already sent message (deleting spaces, re-installing fresh file, even re-installing whole theme) and the problem persists.
I’m able to edit the files with a text editor, but error persists if attempting directly from wordpress. No clue on what the problem could be.
