I’m looking for a plugin or other solution which provides a form solution that asks the user how many people they’re doing something for and then dynamically generates that number of additional fields and allows for calculation of all fields with numbers.

Example:
“How many people do you need to buy gifts for?”
(User enters 5)

5 additional fields are generated which allows them to select relationship to person.
Calculation takes place to determine how much user should pay for each gift.

Rough logic of how everything should occur. Please let me know if you need additional information.

I would love to be able to hook it to a CSV or list of zip codes that the client services to have it return the quote, with zips that are not entered in the list to return “Call for quote.” Would that be possible?
