icon-4 is mislabelled/misnamed and therefore not available.
You can fix it by going into the plugin folder > src_icons and changing Icon-4 to icon-4.

I purchased the premium version of the plugin, installed it correctly, however, it won’t retain any of my settings and always falls back to a very basic, default, kind of behavior.
Please help me sort this out as soon as you can.

I just payed for your premium version of this plugin, but after install, my IE 11 stop working, when I scroll down page and active the “To Top” pop up.
