Please fix the code and ensure that it produces no warnings.

Need a way to change the password reset URL that this plugin automatically generates in the email it sends out. Would love to have a text-box in the configuration options, so that I can customize the password reset URL. Right now the URL it sends out works, but it does not fit with the design of the site.

Need a checkbox added to the plugin – so that we can disable it’s automated functionality on a staging site. It’s been automatically sending double password expiry emails (one from production, the other from staging). And users are getting confused – trying to reset their passwords on the staging site.
