There is an error in function adv_cod_add_payment_gateway_extra_charges_row if extra_charges_type==”percentage”
The amount of charge is always the base charge, even if set in percent mode. Need to correct 2 or 3 lines of code.

It’s really well done, and covers pretty much any strange combination of cases. It may not be visually fancy, but whatever. It gets the job done and it doesnt come short in any way. It exceeds the quality of many WooThemes Premium plugins. And there is really no competition.
