The reference button appears in TinyMCE but does not work when clicked.

WP 3.9 has seen a complete overhaul of the TinyMCE Editor and every plugin that adds buttons or what not to the editor should therefore see an update.
Can we therefore soon expect an update of this plugin?

Hi there,

I’ve successfully installed this plug in (and TinyMCE Advanced just prior). Yet no footnotes button shows up on my visual editor. Even if I go into Settings > TinyMCE Advanced, I see no option for including the button.

With curser at position for desired placement the shortcode is placed at the top of article.
