The “Ship to a Different Address” checkbox remains checked after activating the plugin. I see that there are no updates to the plugin in the last 7 months. Is this still being developed and/or supported?

I was checking out your plugin and found after activating, I am not seeing the check box to select a different shipping address. Can you please provide any suggestions?
