Has some limitation, for example it will not take any type of email extension (@name1-name2.co.uk is not accepted for example), and it allows spam (no limitation per IP or per post, etc), but i like it and it does the job!

This is a superb plugin and it works wonderfully.

This plugin handles a tricky issue for me so nicely that it is actually worth more than it has to offer.

Ok, maybe adding a new feature was not easy, now it works as before (with feedback form disabled). The must could be to have a custom Description Box allowing users to disable specific fields (Email) for anonymous report.

It was simple and fine since last update with new -maybe useless- features, now more options make it unusable on WP 3.6 and it does not works anymore. Keep it simple…

Seems to do what it claims, but you need to place code directly in page.php.

I would order a shortcode(s) that could work on page or posts.
