I am using this in conjunction with the Ninja Forms add-on for Sugar Events Calendar. I wanted to use the same form for multiple events for someone to RSVP to different events.

Rather than making a unique form with form title for each event OR making users tell the form which event they are going to, i let this plug-in do the work for me by inserting the page referrer as the subject line.

Just wanted to collect IP addresses of people submitting forms. This plugin worked for me.
