This plugin is well done and there is a lot of options. But I think it could be also multi-languages.
It’s not very hard to do, I just replaced the lines in the “form.html.php” file like this :

<h3 class='top'>Monthly Donation</h3>

by this :

<h3 class='top'><?php echo _('Monthly Donation'); ?></h3>

and created translation files with poedit (in french). When I will have finished, I will send you all if you want.
