Unfortunately, the plugin “Gutenberg Forms” does not work anymore and there have been no updates for a year. The plugin does not seem to be maintained or updated by its original author. ??
It is not recommended to use this plugin! Make sure to replace it with another contact form plugin.

Thank you for the recent update which I have been looking forward to and thanks for the work you did put in to make the plugin compatible with this latest version of WordPress. I took out some time to try out some of the new features which are really great but here are some challenges I faced:
I do hope all these are resolved soon and once again thank you for all you have done for the community thus far. I look forward to contributing to the Open Source project as well once I have learnt the required skills.

Saw Extendify – Instant delete!
Sneaks unwanted extensions into the editor.
I can understand why some sell to Freemius. But extendify feels so scummy.
Especially when it only is mentioned way down the page, like they don’t want everyone to know where the extendify pest comes from. Took me a while to find the reason.
And that the homepage doesn’t exist any more, doesn’t inspire confidence.

The form building process – right in the editor, or as reusable ones – are awesome.
The Conditions and the Multi Step options are not perfect – not working as expected. That was what I was looking for when installed. The problem is initially selected options as radio buttons or select dropdown. The do not get registered, and it goes undetected until submission, that they have no value. This makes the Conditions not detect their value, and therefore cannot work.
I have a support question for this – so far unanswered.
When fixed – this plugin will get my full five star rating.

Have just found out that Caldera Forms, one of the last decent free form builders, is sunsetting and so need to find something that is competitive and expandable.
Gutenberg Forms seems like a great initiative and it would be great if we can better understand how we can contribute and extend this work – for example develop hooks and other exciting functions.

I think this is an awesome and advance form builder with many capabilities, I was playing with it and I have to say that the best and greatest advantage is the freedom you have to build and tweak what you need.
I am still considering to run it in my business site to handle contact forms, opt ins integrated with MailPoet and some conditional forms.

At the time of writing, Jan 2021, this plugin was able to fully do the job that I needed it to do and I’m grateful for it, just see it as a work in progress and be prepared to accept rough edges. You can manually build forms and they work as advertized. The plugin comes with a number of templates, but at the time of writing most of the templates are not updated to work with the current Gutenberg and will display errors when inserted, you’ll have to build them manually which is not hard. The documentation is comprehensive. Thanks to the author for some solid work.

This plugin is much better than any other forms plugin. Most of it is free, whereas others constantly want to plug their PRO versions. It comes with a full-fledged back end database solution. With Contact Form 7 I need to go through hoops to get that.

I can see a lot of work has gone into this plugin because to design & code something as complex as a Gutenberg forms plugin is no small feat.
Congratulations on what you’ve achieved here and I after only half an hour, I can already see it has more functionality than some “old style” commercial plugins.
I’m experiencing some minor issues like not being able to insert a form from one of the many existing templates the plugin includes but I put that down to the “dynamic” nature of the Gutenberg codebase rather than the plugin itself.

I have setup the form and connected with MailPoet, it works really well ??
The only issues I had were to change the styling of required fields and how to setup a form with multiple columns. For both issues the dev replied very fast and provided the appropriate docs.

Don’t often give reviews, but this plugin is value for money (Ok it is free so who can complain).
But really it is easy to use and offers more functionality as some of the commercial plugins.
Calculation fields are very easy to implement.
In one of the other reviews I saw the lack of documentation. This plugin is so easy to use, that should not really be an issue.

I recommend this plugin for casual users, and each time more to pro users, due to its lightweight size and easy to use. Everything is configured inside the Gutenberg post or page, so there is no need to jump pages defining settings as traditional form plugins do.
There are no tricks: no trial period or need to buy this or that expensive limitation. As the plugin description informs: this is an opensource form builder plugin. Therefore form features are there, as they should be.
The developers have heard my feedbacks.
A very promising initiative in Gutenberg era.
