Hi! Your plugin works very well. The field mapping is intuitive.

One problem… you have a notification displaying upon activation:
Please enter your Entries Importing for Forms Plugin license key to enable all functionality. (Settings)

When I click the X to dismiss the notification, it disappears. But then it reappears when I navigate to a new page.
