I am using the Bitware Easy Form plugin
I have included the shortcode in a link on my page to link to the form.
When I click it to test it, it opens a new page that is blank.
I can’t get the form to work

Tested in WP 4.5 within Tested in WP 4.5 theme, and it distorts the page,no form colour change possible, also giving no possibility entering sender- and confirmation e-mail- fields.
