First of all, you should finally fix that typo: you have two times */ on line 22 and 23 which triggers a fatal error during activation.

The layout of the little box where you can choose the category (when uploading a new image) could be improved: the checkboxes are not aligned with the name of the category.

Could you add an option to create a new category in the little box where one can select the categories? Now you have to go back to first create a new category and then return to select it.

The plugin works well with new images. But if you have a few hundred existing images that you would like to assign to a new category, then you have to do it one by one. Can you add an option to select a number of images and assign a category to them all at once? Just add it as an option to the Bulk Actions. Like this the plugin is rather useless for existing blogs.
