See the support threads. Hasn’t worked since wp 4.1 and no word from the plugin author in 8 months. Don’t waste your time with this.

Don’t be fooled by the fact there have been 11 different variations of the program and it’s still hasn’t passed version 1.0 — this plugin has lots of custom options and a cool shortcode called [no_toc] that can be used to prevent the table of contents from automatically populating on a specific page.
