Detecting form URL and passing it back in the email

I have placed one form on pages for individual attorneys at a law firm. The same form is on all pages. Ideally, I could instruct the form on where to send the email based on the attorney page, but I’d settle for getting the form to pass back the URL. I don’t want to set up individual forms for each attorney.

Thoughts?
