Login form submission taking me to a 404 page

Hello,
I have installed Clean Login plugin and used it’s shortcode in oceanwp modal. You can see the modal in the top bar here. Just click on the “OPEN” on the top menu.
When I submit my login credentials, it takes me to the following URL and shows a 404 error:
https://www.bohubrihi.com/ocean_modal_window/login/
