Can you actually provide me more information how you created your form? Is it custom HTML?

Sorry about to delay, just had time to look at this. Can you give me more context please how you create your form. There are multiple ways to create form using Elementor it seems (https://i.imgur.com/K4BRf8u.png). They support other plugins too (e.g. Ninja Form). If you tell me how you want to implement it, I can help better. Thanks!
