Participants Database, List Participants form how to add more fields to form

I would like to add two more fields to display in Participants List and would like to know how to do that. AS I was searching through the forum, the responses to questions were to put a short code in. I am not sure where to put the short codes, thus can yuo be explicit on where exactly to go to make the modification?
