Best form for advanced use

Hey guys,

Just wondering whether any of you could point me in the right direction here. I need to put together some form functionality and I’m not sure whether any of the existing form plugins are suited to the task.

Basically I need the standard options, and also:

– The ability to include fields dynamically based on what the user has entered in a previous field.
– The ability to export to a CSV file.
– The ability to generate separate pages for each form.

I know that sounds a bit bespoke so it’s probably unlikely there’ll be an off-the-shelf solution. But if anybody has experience working with any similar scripts that could offer the same functionality in PHP, please let me know! Thanks
