Custom membership level prices no longer work

Using the latest versions of EDD and WordPress, the custom membership level prices no longer work due to changes in the way they are now handled in the latest EDD (the interface was redesigned since this plugin was last updated).

The discounts continue to function fine, but it’s not possible to set specific price points based on membership levels. The fields are not output for editing.
