Gift Card Customer Form Not Displaying

Hello. The gift card plugin is no longer functioning properly. There is no longer an option at checkout for the customer to enter the gift recipient’s name, email address, etc.

I haven’t changed any of the settings in my gift card settings. Both the plugin, WordPress, and WooCommerce are all updated to their most recent versions. I am using a child theme, though I do not have a modified “checkout” page in the child theme.

Can you help me resolve this? Customers often purchase gift cards during the holiday season, and this is bad timing for this not to work.
