coockie issue

Hi
i follow your instructions but after the user submit the form which in the pop up it reload again. i enable the “ajax mode” to see if it will fix it but than the submit button not working.the coockie set properly so it should show only once per 10 days.
can you please have a look?many thanks for your help!
