Add a title to uploaded media

I am using the plugin to upload mp3 files.
The short-code on the page that displays the upload form looks like this:
[fu-upload-form post_type="radio-shows"]As a result, the mp3 files uploaded through the form go into the media library with no title (actually, with “no title” as a title), despite the fact that the form has a title field.
How can I make the uploaded mp3 take the text written by the user in the form title field?
