Cannot see job posting form

I recently bought a theme from Theme Forest that includes this plugin and has multiple files in the theme folder for this plugin. I have setup the plugin according to the guide but the page where the [submit_job_form] shortcode is posted only displays a pricing table. If i rename the folder in the theme folder I just get a blank page, even while logged in as an Employer.

I’m just trying to figure out how to get access to the form on that page.
