Problems with Sanitorium theme: contact form & “make an appointment” field

Hi guys!
Please help.
I set up a simple website, but something seems to have gone wrong. In the Contacts menu there is supposed to be a contact form, but I see only this:

and this is how it appears on the site.. ??

Second question. I am using the Sanitorium theme and there is a “make an appointment” button, which is not linked to anywhere. Is it meant this way? How are people supposed to make an appointment. I had the feeling that you click there and get to the contact form. Mistaken?
