Group Appointment & Liability Waiver

We need an appointment-booking solution that allows customers to make ‘group’ reservations with the ability to coordinate liability waivers (electronically signed) from each member along with some basic info like height, weight, age, etc. from each participant. The challenge is making the reservation process as simple as possible for the ‘group leader’ while providing a simple way to notify other members of the group and get them to submit their own info separately, yet when they do it is stored in the same place. Does Bookly provide a way to accomplish this? Thank you.
