Does not work with WordPress 4.7.2

    Unable to click Add Media/Add Form/Forms button in the Page editor. Also, whats the shortcode and how do I activate/use the form I’ve created.

    P.S. The buttons mentioned above work fine when this plugin is Deactivated.
