Option labels not showing

I have added the plugin, but for some reason the labels for the options are not showing. Is there something I am missing?

I can’t get your plugin to work correctly. Also it is in table format which is unattractive and uses too much space. It’s been more than three days since I first asked for help and I’ve heard nothing from you. I can’t wait for you to respond and fix the plugin. I am on a deadline. I have to move on to another plugin.
