How can one add it to menu

Make sure that you have primary menu selected in https://example.com/wp-admin/nav-menus.php?action=locations

You can also try to add <?php echo do_shortcode('[gtranslate]'); ?> into your template header.php which will render the language selector.

With the most recent update there is option to show a floating language selector on top right, top left, bottom left or bottom right of your screen.
