How to add an avatar into a wordpress form.

Hello guyz,
I really need somebody’s direction here, i want to create a form with various form fields that almost every form maker has. But i want my form to have a section where the person filling it can upload there photo (passport size) besides filling other sections. Then at the end i want an option to export my information in excel format if i choose to. Which kind of plugin can do this for me??
