am i missing something? Cant get it to work

Hi, I am hoping to use this form so that when customers request a quote, it automatically sends the url of the page they are requesting it from.

I have setup my contact form, it has name, company name, email and notes. I installed your plugin and enabled it then checked all other boxes except the last two, and i did not fill in the text area. But i am not getting the url in the test email, am i missing something?
