• Here is a flow how I produced this issue.

    1. Add some products to cart.
    2. Forward to cart page, apply a 1 time limit coupon.
    3. Forward to checkout page, click checkout but do NOT pay.
    4. Back to store, and add some products other to cart.
    5. Forward to cart page, apply a 1 time limit coupon, AGAIN.
    6. Forward to checkout page, click checkout but do NOT pay.
    7. Move to my-account/orders page, pay for the order created at step 3.
    8. Move to my-account/orders page, pay for the order created at step 6.

    At this point, a 1 time limit coupon has been applied to more than 1 orders.
    The coupon can not be use anymore.

    Is this normal?

    You can do this as manytimes as you want with the flow I listed.

    Look at this settings: Coupon Settings

    Make sure you have set usage limit per user to 1 as well.

    Yes.
    I pretty sure I have set both “usage limit per user” and “usage limit” to 1,
    but can do same thing as well.

    With that settings your coupons list page in Admin Panel should look as per above link.

    It SHOULD be, but NOT if you follow my steps.
    That is why I started this topic.
