Page changes content based on inputted information

Hi,

I have a few things I need to make work, but not sure, how to go about it. I have created a submission form via JotForm, because I need people to submit video files. I have integrated the form with mailchimp and box, so I can manage the files, as well as have a mailing list ready.

What I need to be able to do is to connect the database with the information and the file to be linked to a page on my WordPress site. When a person receives a link to that page, they will have to input the name of the person that invited them and the unique password they received via email. When they input these two, they will see the video file that was uploaded by the person who invited them.

I am not sure, if it is possible to just link the submit form to a single database that would also be connected to the page and if it is possible to retrieve the files based on the information inputted.
