Numerous Errors on install

I tried your plugin on a test install:

??None of the images that were uploaded that were not attached were found. It showed all images were attached to a post. While I know their is atleast 20 images that are not connected to any post.

Also when I turned the debug on I found the following numerous errors:

Please note I replaced my install path with example for anonymity.
