    Only the top level nav items were visible. No “+” next to them to show there were sub menu items. No tree-like menu was generated.

    This is not plugin problem
    May be:
    1- your style of theme or other plugin conflict with this plugin style
    2- conflict this plugin jQuery with theme or other plugin’s jQuery

    Please check these before rate down to plugin or send question in forum
