Unable to select a Gravity Form

Trying to create a new merge (first time, just installed)

On the “NEW MERGE” page the select drop-down under “Gravity Form” is empty and none of my gravity forms are listed

Therefore I’m unable to proceed further. I have tried in both chrome and safari.

From the first page:
Plugin Name : Gravity 2 PDF
Plugin Version : 1.0.2
System Check
PDFTK Library……………. Pass
Temp Path Present………….. Pass
Temp Path Permission………….. Pass

and from the SETTINGS page:
“Use Our PDFtk Library: TRUE”

and Gravity Forms Version 2.0.5
